SGI-USA Queens Community Center Room Reservation Request
Requests for reservation of space for activities at the SGI-USA Queens Community Center should be submitted at least two weeks prior to the event. Reservations will only be accepted for activities at the chapter or above organizational level. Reservations must be endorsed by a chapter or above level leader. Scheduling of room assignments are subject to the approval of the LIQ Region leadership. After submitting this form you will receive a reply indicating the positive acceptance of the request, declination of the request with a reason, or a proposed alternative if there is a scheduling conflict.
